Data used for your care

The Yorkshire & Humber Care Record is accessible to appropriate health and care staff for the purposes of individual, direct care.

You have a Right to Object to your information being shared. However, in an emergency, health and care staff are legally allowed to access the Yorkshire & Humber Care Record to provide you with safe and direct care. To discuss your Right to Object please call 0113 206 4102 during normal working hours.

Note: There are other ways information is shared across the NHS and social care, for example via the Summary Care Record. As these are all operated and managed separately, you need to raise an objection to each one individually.

Data used for population health management

You are automatically opted-in to sharing your de-personalised data for the purposes of population health management. You have the option of opting out by registering with The National Data Opt Out.‎
